Class of 2017 Final Transcript Request

This form is for current graduates in the Class of 2017.  It allows you to either pick up your transcripts in the Granada Counseling Office or have them mailed (via US Mail).  If you would like your transcripts sent electronically, do not use this form.  Instead, go to the on-line service Parchment at www.parchment.com.  There is a $5 charge for this 24/7 service.

 

Please submit a new form for each transcript request such as Institution, Financial Aid, Scholarships, or if you want an official copy for yourself.  Your first 2 transcripts are free.  Each additional transcript is $5.  Checks payable to the Granada Counseling Department.

 

IMPORTANT - If you would like your final transcript to be mailed, PRINT THIS FORM after you complete it and before you click the submit button. Bring the form to the counseling office by June 8, 2017 with a stamped envelope(s) addressed to your institution(s).  Granada will provide a label for the return address.
